Appointments

New patients can inquire by emailing admin@ccfp.clinic or downloading the intake form from ccfp.clinic. After reviewing the form, the clinic will contact the patient to schedule an appointment. A social history form will also be sent, which must be completed and returned by the end of the business day before the appointment. Incomplete forms may result in rescheduling.

The clinic follows a two-step process for new patients, with the parent(s) or guardian(s) having the first appointment, and the second with the minor within two weeks. This ensures timely treatment decisions.

Follow-up appointments for established patients are scheduled based on therapeutic needs, typically every 2-3 weeks initially, then potentially extending to every 1-3 months. Patients are required to have at least two in-person visits per year, though telehealth is available for convenience.

Appointments can be scheduled through the patient portal, via email, or by calling the clinic. The office is open Monday to Thursday from 10 AM to 4 PM and closed on Fridays, weekends, and holidays.

With permission, your provider may request collateral information from medical providers, family or significant others, to aid in case formulation and diagnosis. We often collaborate with other professionals to develop a treatment plan that fits your individual needs, which may include therapy, medication management or both. If you have a therapist that you would like to continue working with, we will be happy to collaborate with that individual, provided you sign a release of information. There will be a consultation fee associated with collaborations between providers at the discretion of the physicians at CCFP.

Appointment Process

For children, we will meet with the parents alone for the first appointment. This is when we will discuss any concerns in-depth, and we will review family and social history closely. Then at the second appointment, our physicians conduct a clinical interview of your child. Parents/guardians must be available at the end of this appointment so that a treatment plan can be discussed.

For adult patients, who are no longer considered a dependent, the new patient evaluation may consist of just one appointment. We ask that when completing the new patient intake paperwork, the adult patient provides a thorough history of any past treatment and outcomes, in addition to clearly stating their concerns that they would like to be addressed.

To most efficiently utilize the time during the appointments, the providers ask for all forms to be completed no later than the end of the preceding business day. This allows for the provider to review the information and adequately prepare for your appointment.

Cancellations

Because of the nature of our work, a missed appointment or a no-show to an appointment often means that we cannot fill your time slot with someone else who is needing an appointment.

If you are unable to keep an appointment, please give at least 24 business hours advanced notice Monday to Friday 9AM- 5PM (excluding weekends and holidays), otherwise you will be charged a cancellation fee of $75 for the time that was reserved for you (for example, if your appointment is on a Monday at noon, you must cancel by the previous Friday at noon or you will
be subject to the cancellation fee.

Missed Appointments

If you are a “no show” for your appointment, you will be charged the full amount of your appointment. If you are a new patient, please give 72 hours or 3 days advance notice or you will be charged the full fee of the initial appointment.

New appointments require more advanced planning; therefore, we require more notice of cancellation for new patient appointments.

You may cancel your appointment by calling the office and leaving a message or cancelling via the patient portal. If you are late for an appointment, you will be seen for the remainder of your reserved time. You will be responsible for the full session fee
(this includes initial visits.)
