– Initial Evaluation Part II: Psychiatric Diagnostic Evaluation with Medical: 120 minutes $500
– Established Patient Follow Up: 50 minutes $310
– Advance Practice Provider Established Patient Follow Up: 50 minutes $250
– Phone Call Sessions: 20 minutes $100
– Psychological Evaluation testing: 3000
– Advanced Practice Provider Initial Evaluation Part 1: New Patient Office Visit: 120 minutes $400
– Advanced Practice Provider Initial Evaluation Part 2: Diagnostic Evaluation with Medical: 120 minutes $400
– Simple Established Patient Follow Up: Medication Management: 25 minutes $200
– Advanced Practice Provider Simple Established Patient Follow Up: Medication Management: 25 minutes $165
*If any of the above sessions last for an extended period of time outside of the allotted time frame (10-15 mins), there will be an additional charge of $75 in addition to the appointment fee.
Our clinic is self-pay only. We do not accept insurance. However, we do provide a superbill the patient can submit to the insurance company for potential reimbursement once a session is completed.
We provide psychological testing to further identify and clarify specific diagnoses. The results can also be used to assist with treatment planning and recommendations. The provider will help to identify the relative strengths and weaknesses in order to recommend the best interventions and accommodations allowing your child to be well equipped for daily and academic functioning. Results will be provided in 2 weeks from the testing date. Additionally, the parents will receive a one on one session with the provider to thoroughly explain the testing results and answer any questions the parent(s) or guardian(s) may have. If you have questions/concerns for the providers and the response will be more than yes/no and deserves a full conversation, please e-mail admin@ccfp.clinic & a 20-minute phone session may be scheduled for $100.
- Administrative Requests: including any letter or forms to a third party, summary of care, medical record: $100/hour. (This is congruent with TN State Law.)
- Forms will be completed upon request. Please allow 14 days for any administrative requests.

All fees are due at the time of service rendered and are payable by credit card (Visa, MC, Amex, Discover). All patients are required to keep an active credit card on file that you will set up during the intake process. The card on file will be enrolled in autopay, which will charge your card on the day of your appointment. Charges will include and are not limited to missed appointments, phone appointments, late cancellations, administrative work, letters & family conferences. There are no changes to these policies when someone other than the patient is paying for visits. Additionally, paying for visits does not change confidentiality; a patient’s progress, medical record and any privileged information can still only be given with direct consent from the patient.
An account that has an outstanding balance will receive a statement or phone call informing the responsible billing party of the amount remaining. Full payment is due by before any further services can be provided, including refills or any additional appointments. Any accounts with long-standing balances may be sent to a collection agency.
